Colour vision test as required by Maritime and Coastguard Agency

Detailed Description

In Vision Research Laboratories, we conduct colour vision assessment as required by Maritime and Coastguard Agency. You will need to bring with you a referral document, which can be any one of the following;
• an ENG 3 form 
• a letter from the Approved Doctor
• an ML5 form (MSF 4112)
• Application for a seafarer vision test sight test form (MSF 4100) if you have had your visual acuity and Ishihara test conducted by an optometrist

The seafarer will also need to take;
• Any glasses or contact lenses worn for visual acuity (colour correcting lens and glasses are not permitted)
• A statutorily issued form of photo identification document (ID) such as a passport, driving licence or seafarer discharge book.  A photocopy or scanned copy is not acceptable.
